This notice describes how medical information about you may be used and disclosed and how you can get accessto this information.Please review it carefully.
Liquid Sunshine Psychiatric Services PLLC is committed to protecting the privacy of your protected health information (PHI). Federal law requires us to keep your protected health information private. PHI includes any health information, as well as your personal information. We have taken steps to make sure that this information remains private, and is not shared with others. There may be times however when your information will need to be shared. This is illustrated as follows.
1) In the event of an emergency situation, such as a visit to an emergency room and/or hospitalization, PHI may be released to the treating facility/provider in order to allow for continuity of care.
2) In the event of a psychiatric emergency if you verbalize suicidal ideations, Liquid Sunshine Psychiatric Services PLLC will contact necessary authorities, such as 9-1-1 and/or designated crisis responders, in order to plan for your care.
3) In the event of homicidal ideations, Liquid Sunshine Psychiatric Services PLLC will contact the authorities as well as the intended victim.
1) If Insurance is billed they may ask for diagnostic codes and treatment plan, and this information will be shared with them.
Continuity of Care
If you wish to share information between Liquid Sunshine Psychiatric Services PLLC and your primary care provider, or psychotherapist, family member, or whomever you wish, you will be asked to sign a Release of Information (ROI). This will be updated annually.
Access to medical records
Your medical records belong to you, and Liquid Sunshine is happy to provide you with a copy of this record. If a paper copy is provided you will be charged 5 cents per page.
