Our office is located in central Seattle in the Cabrini Tower on Boren and Madison. There is pay parking available in the building, as well as street parking, although this may be limited. Notice of Receipt of Practice Guidelines
I have received a copy of practice guidelines of Liquid Sunshine Psychiatric Services PLLC. If I have any questions or concerns it is my responsibility to ask for clarification. I can ask for clarification at any time during my course of treatment.
Should clinical decision warrant the prescribing of medications you will be given a prescription or medications may be called in to your pharmacy of choice. You are then responsible for filling the prescription. If medications are prescribed the understanding is that the client will take them as they are prescribed. Efforts will be made to find medications that are on your insurance panels formulary guidelines.There may be times when medications that are indicated are not covered by your insurance plan, if this is the case efforts will be made to select a comparable substitute. Controlled substances including benzodiazepines, stimulants, or buprenorphine may be prescribed as a treatment for an illness that you may have. If controlled substances are prescribed please be advised that the WA state prescription monitoring authority website will be verified. If at any time it is discovered that you are receiving controlled substances from other providers, your Liquid Sunshine Psychiatric Services PLLC provider may choose not no longer prescribe this medication for you. If you are given a prescription for a controlled substance it will not be replaced if the prescription is lost or misplaced.
If you are unable to make an appointment please provide 72 hour notice. You will be charged the full price of the appointment if you miss an appointment and/or fail to cancel within the 72-hour window. If you attend an appointment under the influence of a substance, the appointment will be terminated and you will be charged. If you make a threat for suicide the authorities will be notified. If you make a threat towards someone else, that person and the authorities will be notified. If you make a threat towards anyone affiliated with Liquid sunshine Psychiatric services PLLC you will be exited from the practice.
